UK HSE – Case Studies
The UK Health and Safety Executive provides guidance through a number of case studies (from the period 1966 tot 2011) that have been identified as illustrating the importance of the technical assessment criteria in preventing, controlling or mitigating major accidents. Each of the case studies provides: - a brief description of the accident - a summary of the technical measures that were missing or not implemented properly - the source reference(s). A table of case studies and technical measures (PDF) provides a cross index of which technical measures were found to be inadequate in each case study.

The table of case studies and technical measures illustrates the technical measures which failed or were not adequately implemented in each of the incidents, and can therefore be used to identify good examples of the importance of particular technical measures.
